Is there a lease standard document to submit to a Tenant? Also, what type of deposit should we request for breaking a lease? Is there a standard agreement to be given to the tenant for damages, etc?

It sounds like you might need some legal advice and information as a landlord.

Contact the Landlord's Self-Help Centre. Landlord's Self-Help Centre is a specialty clinic operating within the province wide community legal clinic system and funded by Legal Aid Ontario.

Landlord's Self-Help Centre is an incorporated non-profit organization that provides information, assistance and educational programs to Ontario's small scale landlords free of charge.

Find out more on their web site:

I hope this information is helpful. Please let us know if you have further questions and if there is any follow up to your question/situation.
